We don’t wait for performance reviews to give feedback. Managers are taught that one of their responsibilities is ensuring each of their reports is always crystal clear on the 1-3 things they should work on next in their personal development.
Twice per year we evaluate who has grown and/or is crushing it and grant raises. At the moment we don't really do formal job titles, but everyone knows conceptually what their level is.
